Print the name of the trade in which you want to apprentice. 2 Personal Information Print your full name clearly. Put your last name first. The name that will appear on any correspondence or documents issued as a result of your application will appear exactly as you show it on this application. If you change your name in the future, please contact the closest Career Development Centre to update your record. We will use the mailing address you provide on this application for all correspondence you receive from us. If you change your address, please advise the closest Career Development Centre immediately. We will then update our record of your address. 3 Education and Training What was the last grade of school you completed Tell us what the last grade in school was that you completed. We will use this information to determine whether you are eligible to enter an apprenticeship program. You must submit an original transcript of your high school marks. A transcript is an official record of the courses you took and the marks you received in those courses. We need the transcript to ensure that you meet the entry requirements for the trade. In some trades, you may be eligible to enter the apprenticeship program by passing an entrance examination if you do not meet the entry requirements for the trade. Some trades have a compulsory entrance exam. We will tell you if you have to take an entrance examination. If you have completed any formal or technical training in the trade identified in section 1 or in any other trade, please describe it below. List any training courses or programs you have completed in the trade in which you want to apprentice and in any trade in which you have taken training. If there is not enough room, you can list additional courses on a separate page. Include the name of the course, the school where you took the course, the date you started the course, the date you completed the course and the recognition you received for taking the course. The recognition for taking the course may be an attendance certificate, a certificate of completion or some other document that says you took the training. We will use this information to decide if you are eligible for credit for the training you took. You must include a certified true copy* of all the certificates or documents for these courses or programs when you submit this application. List any other post secondary education you have completed. We are interested in knowing what other education you have. This might be courses or programs you took at a technical institute, a private college or institute, a community college, or a university. We will use this information to determine if you are eligible for credit toward advanced standing in your apprenticeship program. 4 Previous Work Experience in the Trade Tell us about the work experience you have in the trade identified in section 1. If you have previously worked in the trade identified in section 1, tell us about that employment in this section. If there is not enough room, use a separate page. We will use this information, providing your employer agrees, to grant you credit toward the on the job training component of the apprenticeship program. Indicate the name of the employer or company for whom you worked, the employer’s address, the date you started and the date you completed employment, and the number of months and hours you worked in the trade. You must submit original or certified true copies* of letters you have from previous employers that include a description of the work you performed in the trade and confirm the number of months and hours of experience you obtained when working for them. Previous work experience or training in the trade If you have worked in the trade in which you are applying to be registered as an apprentice, or if you have training in the trade, or both, you may be eligible to write one or more examinations. These examinations would determine your level of competency in relation to the Alberta apprenticeship program in the trade identified in section 1, and your eligibility for credit toward advanced standing in your apprenticeship program. We will advise you if you are eligible to write an examination. 5 Signature of applicant Make sure that your application is complete and accurate in all details before signing the application. 6 Consent to disclose personal information In this section we are asking you to give us your consent to disclose the personal information you provide on the application form and the personal information we obtain during your apprenticeship program for two reasons that are outside the uses described in the notification on page 1 of the application. These will come into effect only if you move to another province or territory and apply for apprenticeship training in that province or territory, or if you become eligible for an award or scholarship. You do not have to sign this part of the application if you do not wish to give your consent. * A certified true copy is a copy that is certified to be an exact copy of the original. A lawyer or a notary public or a commissioner for oaths can do this for you. AECD 228a (96/09) Information / Instructions and Application to become a registered apprentice 7 To be completed by the employer OR in optional certification trades if you are self-employed, by the person or company with whom you have made arrangements for the supervision of your training In order to become a registered apprentice, someone must agree to train and supervise your apprenticeship training. If you are employed, your employer must fill out this section of the application. If you are selfemployed in an optional certification trade (see next page for list of optional certification trades), you may make arrangements with another company or tradesperson to supervise the on the job training component of your apprenticeship program. Mailing Address We will use the mailing address you provide for all correspondence from Advanced Education and Career Development. If you change your address, please notify the Career Development Centre that is closest to you immediately. Site Address In some cases the location of the business is not the same as the mailing address. We need to know where you are located so that we can visit your place of business. Name of contact person Provide the name of the individual representing the employer or the supervisor who can be contacted about this application and about the applicant’s apprenticeship program. We will direct correspondence relating to this application and all future correspondence from Advanced Education and Career Development to that individual. When did the applicant begin work in this trade for your business or as a self-employed person? We need to know when the applicant began to work in the trade with your business, or when the applicant became self-employed. This will help determine how much credit for on the job training should be granted to the applicant. Based on your assessment of the applicant’s skills and knowledge in the trade, do you recommend that the applicant be granted credit toward the on the job training requirements of the apprenticeship program? How much credit, expressed in months and hours, should the applicant be granted? What are your reasons for making this recommendation? The applicant may have previous work experience in the trade indicated in section 1. If this is so and you believe the applicant has some skills and knowledge in the trade, you may recommend that the applicant be granted credit toward the on-the-job training component of the apprenticeship program. When an applicant receives credit towards on-the-job training, it means that the total length of the apprenticeship program will be reduced. Indicate the credit you are recommending the applicant receive in both months and hours (e.g., 18 months, 2100 hours). Please explain the reasons for your recommendation.
